There are no waiting periods or activation fees, and members receive included preventive services along with 35% off eligible dental treatment.
Preventive Care Membership Plan
$39.99 per Month or $399 per Year
Our Preventive Care Plan supports patients who receive routine dental cleanings and preventive care. Membership includes:
- Two dental exams per year
- Two professional dental cleanings per year
- Two fluoride treatments per year
- Oral cancer screening
- Routine dental X-rays as needed
- One emergency exam with necessary X-rays
- 35% off eligible dental treatment
Periodontal Care Membership Plan
$79.99 per Month or $799 per Year
Our Periodontal Care Plan is intended for patients who require ongoing periodontal maintenance following treatment for gum disease. Membership includes:
- Two dental exams per year
- Up to four periodontal maintenance visits per year
- Two fluoride treatments per year
- Oral cancer screening
- Routine dental X-rays as needed
- One emergency exam with necessary X-rays
- 35% off eligible dental treatment
Membership Without Dental Insurance
A Straightforward Way to Manage Your Dental Care
Our membership plans are not dental insurance. They provide included preventive services and savings directly through Locust Family Dentistry without insurance deductibles, waiting periods, or claim submissions.
Membership plans are nontransferable and nonrefundable and cannot be combined with dental insurance or other discounts.
Membership Plan Exclusions
The 35% member discount applies to eligible dental treatment. Invisalign® treatment and case fees associated with full-mouth rehabilitation are excluded. Additional terms and exclusions may apply. Please speak with our team before beginning treatment to confirm how your membership benefits apply.
